When you book an online appointment you will be offered to fill the new patient form online, we encourage you to fill that form online before appointment, otherwise please arrive 15 minutes early before your appointment time and get a form from the reception and fill that out.

Please do not visit the clinic if you are Covid positive, please wear a mask if you have cough and cold symptoms.

We encourage all new patients to book longer appointments.
